Chronic Obstructive Pulmonary Disease (COPD) Ambulatory Management - Phase I

Physician Subject Matter Expert (SME) required to provide input into the design of a patient-centric ambulatory Population Health Management (aPHM) program focused on the management of patients with COPD. The program will be enabled through the use of the Patient-, Practice-, and Care-Coordinator.Wallets and launched across Solve.Care’s global COPD Care Administrative Networks (COPD CAN). The specific goals of the COPD CAN is to:

• Provide clinical decision support to patients, providers and care teams to support evidence-based care delivery

• Improve clinical outcomes and quality-of-live for individuals with COPD

• Measurably reduce the need for hospital care for COPD exacerbations

The individual selected for this project will be working closely with Dr. David S Hanekom, Solve.Care’s Global Chief Medical Officer in the design of the decision support and care processes for launch in Solve.Cares global COPD CAN.
